The following will help guide you through various scenarios and what to do if you are experiencing symptoms, recently traveled or have been exposed to COVID-19.
If a team member enters an incorrect response in the Survyio screening survey and does not have symptoms or an exposure:
- Proceed to work as scheduled
- Go through the paper screening process with the screeners at the appropriate entry point
- You do not need to call Occupational Health
Vaccinated with Exposure and/or Symptoms
Team members that have received both vaccinations, are more than 14 days post booster, and have been exposed to a COVID positive person no longer have to quarantine. They may work, continuing to mask, monitor, use good hand hygiene, wear face shield or goggles if they are patient facing, and when eating or drinking, must maintain proper social distance.
Team members with symptoms that have received both vaccines, and are more than 14 days post booster, must still complete the Survyio app correctly. Symptomatic team members still need to call Occupational Health, and may be sent for Covid testing.
Team members that are symptomatic should:
- During work hours (7:30-4:00) call your hospital Occupational Health. Lewiston 207-795-2830, Bridgton 207-647-6081, Rumford 207-369-1164
- After hours for all 3 hospitals call 207-577-6494
- Report symptoms, plan on not working if you have symptoms
Symptoms/Travel:
If a team member is exhibiting symptoms and is supposed to be reporting to work:
- Complete the Survyio screening questionnaire and answer the questions appropriately.
- Contact Occupational Health using the appropriate phone number for your location and follow their instructions

- Occupational Health Bridgton: 207-647-6081, Monday-Wednesday, 7:30am – 4pm

If a team member is exhibiting other symptoms, include nausea, vomiting and diarrhea:
- Complete the Survyio screening questionnaire and answer the questions appropriately.
- Call your supervisor to let them know you will not be reporting to work.
- Stay home from work until symptoms have been resolved for 24 hours.
- You do not need to call Occupational Health.
If a team member is exhibiting symptoms and is on vacation/Choice Time or off of work:
- Contact your PCP/Family physician, Urgent Care or ED
If the team member is returning from out-of-state travel:
- Contact Occupational Health using the appropriate phone number for your location and follow their instructions

If the team member thinks they were exposed but has no symptoms:
- Complete the Survyio screening questionnaire and answer the questions appropriately.
