The Central Maine Medication Management Clinic offers specialized support for patients managing chronic conditions through expert monitoring, education, and medication optimization. Our team includes nurse practitioners and registered nurses with advanced training in anticoagulation therapy, hypertension, and lipid management. We work closely with primary care providers to ensure safe and effective treatment plans that reduce risks and improve outcomes. We provide comprehensive anticoagulation services including Warfarin (Coumadin®) monitoring and dosing, as well as guidance for patients using other anticoagulants. A referral is required to establish care. Patients can expect:

• Routine Warfarin/Coumadin monitoring by certified anticoagulation clinicians.

• On-site INR finger-stick testing with same-day dose adjustment.

• Option for venipuncture on-site or at a preferred lab.

• Home INR testing, based on eligibility and insurance coverage.

• Perioperative bridging with Lovenox®, and education on transitioning to novel oral anticoagulants (NOACs) such as Eliquis®, Pradaxa®, and Xarelto®.

In addition, our clinic now offers dedicated services for managing hypertension and cholesterol. Patients referred for these services will benefit from:

• Regular blood pressure monitoring and medication adjustments.

• Lifestyle coaching and education to support heart-healthy habits.

• Lipid panel review and treatment optimization to manage high cholesterol.

• Collaborative care with PCPs to improve cardiovascular health and reduce long-term risks.
