Request Your Medical Records

How To Request Your Medical Records

Maine law ensures that patients have the right to access and request disclosure of their medical information. Under this law, a patient’s medical record must be made available upon receipt of a written request from the patient or the patient’s representative. Any disclosure of medical records must be accompanied by the patient’s consent or authorization. Records of your hospital stay are maintained by the hospital’s Medical Records Department, also known as the Health Information Management Department.

If you have questions or need additional information, please contact us at 207-795-2480, Monday – Friday, 8:00 am – 4:00 pm.

To request your medical records, please follow these steps:

1. Download the Release of Information form:

  • Bridgton Hospital – English | Spanish
  • Central Maine Medical Center – English | Spanish
  • Rumford Hospital – English | Spanish

2. Ways to submit the completed and signed form:

  • Fax it to 207-344-0674
  • Email it to CMME-MedicalRecords@primehealthcare.com

Please note: The patient or the patient’s representative must include a copy of a valid, government-issued photo ID with a signature when submitting the Release of Information Form.


For Attorneys, Law Firms, and Third Parties

Central Maine Healthcare uses ChartSwap, a secure, HIPAA-Compliant online platform, to process requests for medical and billing records. Register and submit requests through ChartSwap. Please note: Medical Records and Billing Records must be requested separately. A valid Release of Information form must be included with the request.

Bridgton Hospital is a 25-bed, not-for-profit critical access hospital (CAH) that serves the people of western Maine. Founded in 1917, Bridgton Hospital provides 24-hour emergency services and features medical and surgical services, intensive care and a variety of other services including oncology, hematology, podiatry, orthopedics, cardiology and diabetes education. The hospital provides a comprehensive range of diagnostic services with a full-service lab and a radiology department boasting 3D mammography, ultrasound, CT scan and MRI.

Transition to Epic Electronic Medical Record (EMR) System

We are pleased to inform you that Central Maine has transitioned to the Epic Electronic Medical Record (EMR) system. This upgrade is part of our commitment to enhancing the quality, efficiency, and coordination of care for our patients.

Key Information for Patients

  • Transition Timeline
    Our transition to Epic is currently underway. During this period, some processes may change as we implement the new system.
  • Access to Existing Patient Portal Data
    Your historical health information from our previous EMR system will continue to be available through the MyHealthLink patient portal. You can still log in to MyHealthLink to view past records, lab results, and documents. Your MyHealthLink account can be accessed here.
  • New Epic MyChart Account Required
    After your first visit in our new Epic system, you will need to create a MyChart account, Epic’s patient portal, to access your ongoing medical information.
    MyChart will allow you to:
    • View test results and visit summaries
    • Communicate with your care team
    • Manage appointments
    • Access billing information
  • Important Reminder
    MyHealthLink and MyChart are separate systems.
    • Continue to use MyHealthLink for your past records.
    • Use MyChart for all new health information moving forward.

After your initial visit in the new system, you will receive instructions on how to securely set up your MyChart account. Our staff will be available to assist you if needed.
