Important Update About Your Care

Gray Family Health Center will be closing permanently on March 6, 2026. We are deeply grateful for the trust you’ve placed in us. While this change may be unexpected, we are committed to supporting you through the transition. Please read the following important notes regarding your future care and see the letters that were sent to your home address.

Gray Family Health Center will continue to offer care through March 6, 2026. After that date, patients will need to continue to see their primary care provider at their new location or establish care with a new primary care provider. Amy Bergeron, DNP, FNP-C and Manju Hilary, MD, will be relocating to Topsham Family Medicine, located at 4 Horton Pl. in Topsham. If you have an upcoming appointment with these providers after March 6, we look forward to seeing you at their new location.

Ben Slocum, DO, will be leaving our practice on March 6. Please join us in wishing Dr. Slocum the best in his future endeavors.

Our team is confident you will feel comfortable and secure with another provider on our care team, and we are committed to seamlessly transitioning your care. If you have an upcoming appointment with Dr. Slocum after March 6, our team is available to support you in finding a new provider by calling 1-866-834-8343.

Please note that any testing or lab orders after March 6 will need to be signed by your new provider (unless you continue to see your current provider at their new location), and all results will be delivered directly to their office. If you are awaiting test results after the closure date, please contact your new primary care provider to follow up. Accessing Your Medical Records Your medical records will remain available after the closure. To request your records, please complete an authorization form, which you can find on our website or request by calling (207) 795-2480. ________________________________________
