Note Regarding Manchester Care Center Manchester Care Center will continue to offer care through October 31, 2025. After that date, patients will need to establish care with a new primary care provider.

Please note that any testing or lab orders after October 31 will need to be signed by your new provider, and all results will be delivered directly to their office. If you are awaiting test results after the closure date, please contact your new primary care provider to follow up.
Note Regarding Therapy and Lab Services As part of the Care Center closure, therapy and lab services will also end on October 31, 2025. Until then, you may continue your therapy sessions and lab services as usual.
If you have therapy scheduled after the closure date, you’ll need to obtain a new referral and evaluation with another provider. Please speak with your current therapist to discuss your ongoing care needs and identify the best next steps.

Your medical records will remain available after the closure. To request your records, please complete an authorization form, which you can find on our website or request by calling (207) 795-2480.
