Lewiston/Auburn Community Health Implementation Plan

Central Maine Healthcare (CMH), through its three hospitals, Central Maine Medical Center, Bridgton Hospital and Rumford Hospital, contributes to our communities throughout the region by analyzing the particular needs of those we serve and developing and delivering programs to meet those needs. In the spring of 2019, CMH implemented a Community Health Needs Assessment. Through this process, we identified the top health needs, as determined by community members, and developed three-year plans for each hospital to meet those needs.

About the Maine Shared CHNA

The Maine Shared CHNA (MSCHNA) is a dynamic public-private partnership that creates shared Community Health Needs Assessment reports, engages and activates communities, and supports data-driven health improvements for Maine people.

The vision of the Maine Shared CHNA helps to turn data into action so that Maine will become the healthiest state in the U.S. The Maine Shared Community Health Needs Assessment is a collaborative effort between Central Maine Healthcare (CMH), Northern Light Health (NLH), Maine General Health (MGH), Maine Health (MH), and the Maine Center for Disease Control and Prevention (Maine CDC).  This unique public-private partnership is intended to assess the health needs of all who call Maine home. CHNA State and District Reports

All of the 16 County Reports are now all available on the Maine CHNA website. Visit www.mainechna.org, and click on “Final CHNA Reports” to view the reports.

  • County, Public Health District, State, and selected City Data Health Profiles, and;
  • Health Equity Data Sheets on Race, Sexual Orientation, Sex, Education, Ethnicity, and Income
